Description
PESA (percutaneous epididymal sperm aspiration) is a simple procedure performed when a blockage is preventing sperm from being released naturally, for example is there has been a vasectomy. The area is numbed, and a small sample of fluid is taken which is checked for healthy sperm. This can be used in IVF or ICSI treatment and if suitable, it can sometimes be frozen for future treatment.

MESA sperm retrieval
Description
MESA (microsurgical epididymal sperm aspiration) is a precise procedure to collect sperm when an obstruction stops sperm from being released naturally eg after a vasectomy. Using an operating microscope, a tiny opening is made and fluid is gently removed that contains good quality sperm. Any sperm found can be used for IVF or ICSI, and is usually frozen for future treatment.

Sperm retrieval – multiple open testicular biopsy
Description
This surgical procedure is used to look for sperm directly inside the testicles, when it’s not not being released. Carried out as a day case at the Royal Victoria Infirmary under general anaesthetic, the procedure takes tiny samples of tissue which are examined for sperm. This can be used for IVF or ICSI and can often be frozen for future treatment. If no sperm is found, the surgeon takes biopsies from each testicle.

MicroTESE sperm retrieval
Description
MicroTESE (microsurgical testicular sperm extraction) is a specialist procedure used when sperm isn’t being made. The procedure is done as a dayâcase under a general anaesthetic at the Royal Victoria Infirmary. A small opening is made and a microscope is used by the surgeon to search carefully inside for the most promising tissue. Samples are taken straight to Newcastle Fertility Centre so scientists can look for sperm suitable for ICSI. The opening is closed with dissolvable stitches. Any sperm found may not freeze well, so the female partner usually has IVF at the same time so that fresh sperm can be used immediately. Some couples choose to have donor sperm available as a backâup, and counselling is offered to support this decision. Everyone providing sperm or embryos must be screened for HIV and hepatitis B and C. At present, we are unable to process samples from patients who test positive, so surgical sperm retrieval cannot go ahead in those cases.
