Please read this page and tick the box in the agreement of services to opt in, or out, of your personal data being shared with the PHIN.
As part of the acute data alignment programme (ADAPt), Newcastle Hospitals Private Healthcare service will share your personal data and NHS Number in England and Wales, CHI Number in Scotland or Care Number in Northern Ireland, with the Private Healthcare Information Network (PHIN).
Sharing of your personal data will only take place with your explicit consent. If you choose to be excluded from this sharing, Newcastle Hospitals Private Healthcare service is still required to share your NHS number to the PHIN.
Your consent for your personal information to be processed in this way is entirely voluntary. You do not have to give consent and are free to withdraw consent at any time without giving any reason, and without your medical care or legal rights being affected.
PHIN will further share only your NHS Number to NHS England, who will use this information to link to national hospital and mortality datasets. Your information will not be shared or analysed for any purpose other than those stated above by the PHIN.
The purpose of linking your information by NHS England aims to improve the public’s access to information on the quality and outcome of private healthcare and is used by the PHIN to measure quality of care and to check for adverse events after discharge from our hospitals, such as unplanned readmissions to hospital, emergency transfers between hospitals or deaths following treatment.
All information about you will be treated with highest standards of confidentiality and in accordance with data protection laws and the duty of confidentiality by the PHIN. Any information that is published by the PHIN or NHSE will always be in anonymised statistical form.
