- Newcastle Hospitals Private Healthcare is part of The Newcastle upon Tyne Hospitals NHS Foundation Trust (the trust).
- The trust reserves the right to ask for a full deposit or seek authorisation or confirmation of payment by an insurance company/ guarantor or other third-party funder prior to the commencement of any episode of care. It also reserves the right to charge a cancellation fee if you cancel any treatment or procedure within seven days or less of your scheduled admission/test date.
- The trust’s terms of payment are 30 days from date of invoice. If you have medical insurance or someone else is paying, it is your responsibility to confirm with your insurer or other funder/guarantor, before treatment commences, that you are fully covered for the treatment you are receiving. The private patient team will contact your insurer to confirm the authorisation is in place. If the episode of care or any part of the treatment is not covered by your insurance policy or if for any reason your insurer, or other third-party funder/guarantor refuses to pay or authorise treatment of any kind wholly or in part, you will be liable for all or part of the hospital charges and any professional fees. If insured and the treatment authorised, the trust will send your invoice direct to your insurer. If you would like a copy, please contact the trust’s finance office at Regent Point, Gosforth.
- I understand that if all attempts of settling the account with the third party or insurer fails, that I am responsible to pay any amounts outstanding due to the trust. I understand that if I do not pay an invoice on time, I may be charged interest and/or late payment fees, and the trust could also pass the matter onto a collection agency who could charge me additional fees. If the matter is passed to a collection agency, I hereby give my consent for the trust to pass this form to the collection agency. If you are self-funding your healthcare, then payment will be required in advance.
- If you are self-paying for your treatment or tests, you will be required to pay the deposit amount or fixed cost price in advance of your treatment or tests starting (usually 7 days prior to admission/attendance). Failure to pay may result in your treatment or tests being cancelled. Please contact the private patient team for more information about this if necessary.
- I, the above named, accept responsibility to pay full charges to The Newcastle upon Tyne Hospitals NHS Foundation Trust for accommodation and services given to myself/the patient named on this form as a Private or non-NHS Patient. I understand that such charges will cover out-patient/diagnostic procedures, treatment, drugs, inpatient accommodation, and all items, including appliances or devices, which may be required as part of the service.
- I also authorise disclosure of any medical notes including the provision of copies thereof to my insurer or for continuing healthcare as part of their claim and payment procession requirements. This right also includes overseas transfer, since as part of your continuing care we may need to send your data to agencies overseas.
- You understand that your consultant(s) are working in a self-employed capacity and provide their services direct to you and not on behalf of The Newcastle upon Tyne Hospitals NHS Foundation Trust
